Goal-Based Services
Youth who must master specific objectives in order to transition successfully back to their family and community are appropriate for Goal-Based Services. These youth typically have a combination of individual needs, dysfunctional family dynamics, and available supports or therapeutic services in their hometowns have not been successful. This service recognizes that individualized skill development, opportunities for practice and continued support is necessary for a successful outcome.
Treatment is based on the following:
- Placement was initiated on the basis of specific needs demonstrated by student and family
- Treatment goals are developed based on those identified needs
- Achievement of those goals is the criteria for discharge readiness
- Moderate support will be required in the community to maintain and promote progress
From the identified treatment goals, an Individualized Service Plan (ISP) is developed for each youth with specific objectives, and treatment is initiated.
Services include:
- Developmental-behavioral support program
- Trauma-informed care recognizing triggers and fostering the youth’s awareness and skill in identifying, understanding, tolerating, and managing their internal experience
- Family engagement to build healthy, supportive relationships and improve family function
- Transitional services for continued success post-discharge
- Clinical services providing individual and group counseling and if appropriate, substance abuse groups
- Educational services in supportive, structured setting
- Medication management
- Aftercare recommendations
